Care Coordination Request
The Care Coordination Request process is to be used by an agency when all other activities to find services to address the needs of the client has been exhausted. It is not meant to replace the Coordinated Entry process for the Muskegon County Homeless Continuum of Care Network. Coordinated Entry is a process developed by the Network to ensure that all people experiencing a housing crisis have fair and equal access and are quickly identified, assessed for, referred, and connected to housing and assistance based on their strengths and needs.
The Client must sign the Release of Information form that has been approved by the Community Coordinating Council and is found on the website along with the Care Coordination Request form.
Please scan both completed forms and send to either Alicia Cox at: [email protected] or Jana Routt at: [email protected]. Ms. Cox and Ms. Routt are the Co-Chairs of the Case Managers Committee. Either Ms. Cox or Ms. Routt will contact the agency requesting the care coordination process to schedule a meeting to provide additional guidance in finding community resources to assist the client.
