Care Coordination Request
The Care Coordination Request process is to be used by an agency when all other activities to find services to address the needs of the client has been exhausted. The Client must sign the Release of information form that was approved by the Community Coordinating Council in March. 2015 and is found on the website along with the Care Coordination Request form. Please scan both completed forms and send to either Tressa Crosby at: email@example.com or Jennifer Brennan at: firstname.lastname@example.org, the Co-Chairs of the Case Managers Committee. Either Ms. Crosby or Ms. Brennan will contact the agency requesting the care coordination process to schedule a meeting to provide additional guidance in finding community resources to assist the client.
