(Homeless Prevention and Rapid Re-Housing Program – Recovery Act)

The HPRP program assists individuals and families in two areas: Prevention and Rapid Re-Housing.
Prevention: individuals or households at or below 40% AMI and that have an actual or pending eviction notice.
Rapid Re-Housing: individuals or households at or below 30% AMI and is currently homeless (i.e., residing in shelter, sleeping in a place not meant for human habitation, exiting institution following homelessness, exiting Transitional Housing, or Fleeing a Domestic Violence situation.)

ACCESS FOR COMMUNITY AGENCIES
As of February 2010 the MCHCCN has agreed to use the Community Case Coordination Form as a referral method for potential HPRP applicants (please see attachment below)
Appointments are scheduled beginning each Monday morning at 9am – calls and referrals will be taken until capacity is reached each week. Once at capacity, scheduling will begin again the next Monday morning at 9am.
Walk ins, faxes, mailing or dropping off of the referral form is currently not accepted – all referrals must follow the guidelines listed on the Community Case Coordination Request form for consideration – thank you!
