Transition Grant Materials

The information below is for agencies that currently receive Permanent Housing (Permanent Supportive Housing, Rapid Rehousing, and Joint Component Transitional Housing-Rapid Rehousing) funding in Muskegon County.

Given the shift in HUD’s funding priorities in the FY2025 CoC Competition, the CoC will need to reallocate a significant portion of the funding currently used to fund Permanent Housing projects.

Current recipients of CoC Permanent Housing funding are asked to complete this application form to indicate their interest in transitioning current Permanent Housing funding to another type of program. Additional details are provided in the document.
